Sponsorship Program and Pledge Form
This file is downloadable as a PDF file format. One side of this form is the sponsorship program categories and the other side is the pledge form. Please fax us the pledge form, send it by regular mail, or scan it and send it to us electronically. Once we have the form, we will invoice you for the amount you have agreed to and signed by your organization or as an individual.

This is a form that you may submit on-line to indicate your interest to support CMLI by way of a sponsorship. Prior to completing this form please ensure that you have read the sponsorship categories and understand the conditions of sponsorship. Thank you!
